สำหรับการนำข้อมูลส่วนบุคคลมาใช้สำหรับการลงทะเบียน ตรวจสอบสิทธิในการเข้าพื้นที่การประชุม AM2026
1. ข้าพเจ้าตกลงให้ความยินยอมให้สำนักงานเศรษฐกิจการคลัง เก็บรวบรวม ใช้ และเปิดเผยข้อมูล ส่วนบุคคลของข้าพเจ้า ตลอดจนข้อมูลส่วนบุคคลที่มีความอ่อนไหวสูง ได้แก่ ภาพถ่ายใบหน้า หรือข้อมูล ที่บันทึกอยู่ในหน่วยความจำหรือระบบอื่นใดในบัตรประจำตัวประชาชน ได้ทุกเมื่อตลอดระยะเวลา การประชุมประจำปีสภาผู้ว่าการกองทุนการเงินระหว่างประเทศและกลุ่มธนาคารโลก ปี 2569 The 2026 Annual Meetings of the International Monetary Fund and the World Bank Group (AM2026) เพื่อประโยชน์ในการใช้ลงทะเบียน ตรวจสอบสิทธิในการเข้าพื้นที่การประชุม AM2026 และยืนยันตัวตน รวมถึงการตรวจสอบประวัติอาชญากรรม และนำไปประมวลผลและการดำเนินการ เพื่อประโยชน์ในการจัดการอื่น ๆ ที่เกี่ยวข้องทั้งระหว่างการประชุม AM2026 และภายหลังจากการประชุม AM2026 สิ้นสุดลง 1 ปี (เดือนตุลาคม 2027)
I hereby consent to the Fiscal Policy Office to collect, use, and disclose my personal data, including highly sensitive personal data consisting of facial photographs or information stored in memory or any other system in the national identification card, at any time throughout the period of The 2026 Annual Meetings of the International Monetary Fund and the World Bank Group (AM2026) for the purpose of registration, verifying eligibility for access to the meeting areas of the AM2026, identity verification, including criminal record checks, and for processing and other related administrative purposes during the AM2026 and for one year after the end of the AM2026 (October 2027).
ข้าพเจ้ารับทราบว่าข้อมูลส่วนบุคคลของข้าพเจ้าที่ได้ให้ความยินยอมในการเก็บรวบรวม ใช้ และเปิดเผยนั้น มีรายละเอียดเกี่ยวกับประเภทและรายการข้อมูลส่วนบุคคล ดังนี้

I acknowledge that the categories and details of personal data for which I have given consent to be collected, used, and disclosed will be used to achieve the purposes of registration, verification of eligibility to access the AM2026 meeting areas, identity verification, including criminal background checks, and other related administrative purposes during the AM2026 and for one year after the end of the AM2026 (October 2027).

2. ให้บริษัท เอ็น.ซี.ซี. แมนเนจเม้นท์ แอนด์ ดิเวลลอปเม้น จำกัด (NCC) และสำนักงานตำรวจแห่งชาติ เป็นผู้เก็บรวบรวม ใช้ และเปิดเผยข้อมูลส่วนบุคคลของข้าพเจ้า ตลอดจนข้อมูลส่วนบุคคลที่มีความอ่อนไหวสูง ได้แก่ ภาพถ่ายใบหน้า หรือข้อมูลที่บันทึกอยู่ในหน่วยความจำหรือระบบอื่นใดในบัตรประจำตัวประชาชน ตามคำสั่งหรือในนามของสำนักงานเศรษฐกิจการคลัง ได้ทุกเมื่อตลอดระยะเวลาการประชุม AM2026 เพื่อตรวจสอบสิทธิในการเข้าพื้นที่การประชุม AM2026 ยืนยันตัวตน รวมถึงการตรวจสอบประวัติอาชญากรรม และการดำเนินการเพื่อประโยชน์ในการจัดการอื่น ๆ ที่เกี่ยวข้อง
I hereby consent to allow N.C.C. Management and Development Co., Ltd. (NCC) and the Royal Thai Police to collect, use, and disclose my personal data, including highly sensitive personal data such as facial photographs or information stored in memory or any other system in the national identification card, under the instruction or on behalf of the Fiscal Policy Office throughout the AM2026 period. This is for the purpose of verifying eligibility for access to the AM2026 areas, identity verification, criminal background checks, and other related administrative purposes.
ความยินยอมของข้าพเจ้าข้างต้นให้มีผลไปตลอดช่วงระยะเวลาการประชุม AM2026 เว้นแต่ การให้ ความยินยอมในกรณีการนำข้อมูลไปประมวลผลและดำเนินการต่าง ๆ เพื่อประโยชน์ในการบริหารราชการแผ่นดินในอนาคต ข้าพเจ้ายินยอมให้มีผลต่อไป แม้ว่าการประชุม AM2026 จะสิ้นสุดลงแล้วและภายหลังจากการประชุม AM2026 สิ้นสุดลงใน1 ปี (เดือนตุลาคม 2027)
My consent shall remain valid throughout the duration of the AM2026. However, for the processing and use of data for public administration purposes in the future, my consent shall remain valid even after the end of the AM2026 and continue for one year after the end of the event (October 2027).
ข้าพเจ้ารับทราบว่า ในฐานะเจ้าของข้อมูลส่วนบุคคล ข้าพเจ้ามีสิทธิตามพระราชบัญญัติข้อมูลส่วนบุคคล พ.ศ. 2562 ดังต่อไปนี้ (ก) สิทธิเพิกถอนความยินยอมตามข้อ 1 และข้อ 2 (ข) สิทธิขอเข้าถึงและขอรับสำเนาข้อมูล ส่วนบุคคล (ค) สิทธิขอให้ส่งหรือโอนข้อมูลส่วนบุคคล (ง) สิทธิขอให้ลบ ทำลาย หรือทำให้ข้อมูลส่วนบุคคล ไม่สามารถระบุตัวตนผู้เป็นเจ้าของ (จ) มีสิทธิขอให้ระงับการใช้ข้อมูลส่วนบุคคล (ฉ) สิทธิขอแก้ไขข้อมูลส่วนบุคคล ให้ถูกต้องเป็นปัจจุบัน สมบูรณ์ และไม่ก่อให้เกิดความเข้าใจผิด (ช) สิทธิร้องเรียนกรณีที่มีการฝ่าฝืนหรือไม่ปฏิบัติตามพระราชบัญญัติข้อมูลส่วนบุคคล พ.ศ. 2562 โดยการใช้สิทธิดังกล่าว จะต้องแจ้งความประสงค์ในการใช้สิทธิเป็นหนังสือมายังสำนักงานเศรษฐกิจการคลัง
I acknowledge that, as a data subject under the Personal Data Protection Act B.E. 2562 (2019), I have the following rights: (a) Right to withdraw consent under clauses 1 and 2 (b) Right to access and obtain a copy of personal data (c) Right to data portability or request transfer of data (d) Right to request deletion, destruction, or anonymization of personal data (e) Right to restrict processing of personal data (f) Right to correct or update personal data to ensure accuracy and completeness (g) Right to lodge a complaint in case of violation of the Personal Data Protection Act B.E. 2562. To exercise these rights, a written request must be submitted to the Fiscal Policy Office.
ข้าพเจ้ารับทราบว่า ในกรณีที่ข้าพเจ้าใช้สิทธิเพิกถอนความยินยอมตามข้อ 1 และข้อ 2 หรือใช้สิทธิ ขอให้ลบหรือทำลายข้อมูลส่วนบุคคลของข้าพเจ้า หรือทำให้ข้อมูลส่วนบุคคลของข้าพเจ้าไม่สามารถระบุตัวตนผู้เป็นเจ้าของ ข้าพเจ้าจะถูกระงับสิทธิในการลงทะเบียนและการเข้าพื้นที่จัดงานประชุม AM2026
I acknowledge that if I withdraw consent under clauses 1 and 2, or request deletion, destruction, or anonymization of my personal data, I will be suspended from registration and access to the AM2026 event area.
ทั้งนี้ ให้ถือว่าคู่ฉบับและบรรดาสำเนา ภาพถ่าย ข้อมูลอิเล็กทรอนิกส์ หรือโทรสารที่ทำขึ้นจากหนังสือ ให้ความยินยอมฉบับนี้ เป็นหลักฐานในการให้ความยินยอมของข้าพเจ้า
Copies, duplicates, electronic records, or facsimiles of this consent form shall be considered valid evidence of my consent.
ข้าพเจ้าขอรับรองว่าข้อมูลที่ข้าพเจ้าได้ให้ไว้ในการลงทะเบียนเข้าร่วมการประชุม AM2026 ถูกต้อง ตามความเป็นจริงทุกประการ และรับทราบว่าการนำเข้าสู่ระบบคอมพิวเตอร์ซึ่งข้อมูลอันเป็นเท็จเป็น การกระทำความผิดตามกฎหมาย หากข้อมูลดังกล่าวไม่ถูกต้องตามความเป็นจริง ข้าพเจ้าตกลงยินยอมให้หน่วยงานที่เกี่ยวข้องดำเนินการระงับสิทธิในการลงทะเบียนและการเข้าพื้นที่จัดงานประชุม AM2026
I hereby certify that all information provided for the AM2026 registration is true and correct in all respects. I acknowledge that entering false information into a computer system constitutes an offense under applicable law. If any information is found to be inaccurate, I agree that the relevant authorities may suspend my registration and access rights to the AM2026.
